Shared Decision Making and Court Ordered Treatment: Lessens Learned After a Year

In 2017, Maricopa Integrated Health System (MIHS) in Arizona, developed the First Episode Center, a coordinated specialty care team for young people with their first episode of a primary psychotic disorder. MIHS operates a large safety net hospital system and primary care centers, as well as serves as the involuntary hospital system for people who are evaluated for forced mental health treatment. Most of our referrals have come from our involuntary inpatient units, which then requires us to monitor the outpatient treatment for one year. The evidenced based principles of this early intervention program are person centered care, recovery, and shared decision making, which are in direct opposition of mandatory court ordered treatment. As well, the MIHS First Episode Center was developed to match the traditional Arizona case management model with guidance from the OnTrack New York model, which is more therapy based. Over the year that our program has been running, we have learned to use different techniques to blend the oppositions and help our young people move forward with their brain health, their goals, their treatment, and instill hope for their future.
